Gustavus Health Service can provide accommodation documentation for students who are receiving active, ongoing treatment. It is expected that the patient is well known to the medical provider, having met with them enough to know the patient is following recommended treatment plans and appropriate follow up. Accommodation documentation can only be recommended as an essential part of a treatment plan designed to address a disability associated with diagnosed medical or psychiatric condition and is always based on the provider’s clinical judgement. Gustavus Health Service is not able to provide documentation for new patients. 

If you are seeking accommodation documentation please schedule an appointment with your provider. Please bring all completed paperwork to your scheduled appointment. A Release of Information will be needed from the office requesting the documentation prior to any information being provided.
