Required Immunizations/Vaccines

Minnesota law requires all students born after Dec 31, 1956 who enroll in a Minnesota college or university to be immunized against diphtheria, tetanus (Td or Tdap), measles, mumps and rubella (MMR). This law allows for some exemptions. Additionally, Gustavus will require all students to be immunized against COVID-19. Gustavus students must upload their immunizations and/or the appropriate exemption documents at the start of their college career. If you do not complete the requirement by the 10th day of class, a registration hold will be placed on your record for the following semester. The easiest way to meet the requirement for first years is to log into your Gustavus Health Portal and complete the immunization form.

The Gustavus Health Service automatically receives record of vaccinations administered in Minnesota through the state's secure immunization records database. Upperclass students who received their COVID-19 vaccine somewhere besides Minnesota should upload their COVID-19 vaccine documentation to the Gustavus Health Portal.

For those who are looking for a COVID-19 vaccine in Minnesota please visit our Vaccine Resources webpage.

Students claiming Medical Exemption must print the Gustavus Immunization Exemption form which must be signed by a healthcare provider (MD, DO, PA, or NP). Your immunization requirement will not be fulfilled until this signed form is uploaded to your Gustavus Health Portal.

Students claiming Conscientious/Religious Objection must print and complete the Gustavus Immunization Exemption form, which must be notarized. Your immunization requirement will not be fulfilled until this notarized form is uploaded to your Gustavus Health Portal.
