Cooperating Teachers/College SupervisorsInformation and Forms
This page is for cooperating teachers and college supervisors working with Gustavus Student Teachers. Thank you for all you do! Here are the forms you’ll need to fill out during your student teacher’s placement. Required Documents from the Cooperating Teacher
At the end of the field placement, Cooperating Teachers will need to have completed all forms online, and provide a printed copy of the SEP to the college supervisor at the final student teaching conference. Most forms can be submitted online in the links provided below. The Department of Education automatically receives the online forms submitted so no need to reprint and turn in (unless you will need a hard copy to have a conversation with). The letter of recommendation cannot be submitted online but will need to be submitted to the Education Department. - Assessment Standards of Effective Practice (SEP) is to be filled out online at the end of the placement. This assessment should be discussed with the student teacher. This form is available here: https://gustavus.edu/forms/mn-standards-assessment/.
- Evaluation of the Student Teacher Program is to be filled out online at the end of the placement. This evaluation form can also be submitted online here: https://gustavus.edu/forms/coop-teacher-eval/. (Not required for J-term student teachers)
- Cooperating Teacher Background Information is to be completed by the end of the placement but can be filled out at any time. It is used to provide the college with the cooperating teacher’s background, strengths and areas of expertise. This form can be submitted online here: https://gustavus.edu/forms/coop-teacher-info/.
- Letter of Recommendation must be typed on school letterhead, your office can help you with this.
Please send or email to Vicki Fischer, vfischer@gustavus.edu in the Education office. It will be placed in the student teacher’s job file and used in the job search process. Suggestions for writing letters of recommendation can be found in the Student Teaching Handbook. - W9 form, which are needed to be on file in our Finance Office so that we can process your stipend, can be found here. Just the first page is needed, dated and signed.
Required Documents from the College Supervisor
Professionalism Assessment (Online submission) (If you have access to Taskstream - fill it out there) - at the end of the student teaching semester (not for J-term student teachers)
Diversity Awareness Assessment (Online submission) (If you have access to Taskstream - fill it out there) - at the end of the student teaching semester (not for J-term student teachers)
Student Teaching Observation Reports - Duplicate forms can be found in the Education Department Office or College Supervisors may type their own form of observation notes.
Letter of Recommendation (See #5 in Cooperating Teacher Forms list for directions about letterhead)
Documents from Cooperating Teacher (obtained at the 3 way conference with Student Teacher, Cooperating Teacher, and Supervisor if CT hasn't completed the forms online - please see list above)
