How to sign up: 

• Go to the Gustavus Health Portal and sign in.
• Go to appointment scheduling on the top of the page.
• You must choose the date first  (10/11/22 or 10/12/22) and then choose Flu Shot Clinic. Available times will appear.  

Prior to your appointment:

• Complete the Vaccine Consent Form within the health portal.  
• Upload a copy of your insurance within the health portal or email an image to rwelp@gustavus.edu.

**COVID-19 Bivalent vaccines will also be available, while supplies last.
