In an effort to increase clarity and reduce paper flow, students will download and submit forms online on the Center for International and Cultural Education Form Storage website.
Please read the following information carefully before using the link below to take you to the forms.
Downloading a Form
- Log in to the Center for International and Cultural Education Form Storage website (below) using your Gustavus username and password.
- On the "Download a Form" tab you will find a list of the forms you will need to complete and submit.
- Click on each form to download it, and save it to your desktop.
- If a form is not saved, it will not contain any of the information you have filled out.
Filling Out a Form
- Every form has portions which must be filled out on your computer.
- Some of the forms require you to fill out all fields on the form on your computer using Word, then submitting it on the form storage website (see below). Do this with the following forms:
- Emergency Information Form
- Medical Report Form
- Medical Self-disclosure
- Policy on Alcohol and Illicit Drugs
- Some forms require you to fill out the form on your computer using Word, then printing, signing, and handing it in to the Center for International and Cultural Education. Make sure you fill out everything except your signature on your computer before printing the form. Then sign it. Do this with the following:
- GAC Agreement Form
- GAC Waiver
- Honor Code
- Student Status Form
Submitting a Form Online
- When you have completed a form, go to the Center for International and Cultural Education Form Storage website and log on using your Gustavus username and password.
- Click on the "Submit a Form" tab.
- From the drop-down menus, select which form you are submitting and the course to which you have been accepted.
- If there is anything you would like to note about the form, you may write this in the box provided.
- Click the "Browse..." button. A dialog box will open. Find the correct form on your desktop and click "Open". You should see the file name in the File box.
- Click the Save button. If done correctly, a message will appear saying that the file was stored correctly.
- You can see which forms you have submitted by clicking on the Submitted Forms tab.
- Only submit the four forms that are to be turned in online (see above).
All forms must be submitted by May 27, 2009
When you have read the information above, you are ready to go to the Form Storage website.
